Treatment Summary

Once treatment is completed your clinician will complete a treatment summary.

This is a short document completed by your Consultant which states information about the cancer, its side effects and any potential treatment, side effects of treatment or signs and symptoms of recurrence. It is then shared with the patient and their GP to inform them of any actions required or who to contact with questions or concerns. The GP can then update their records and use the document as a basis for conducting a Cancer Care Review.

Treatment Summaries can be completed manually on paper or electronically on hospital clinical systems.
